Dong Quai (Angelica sinensis) is a classic herbal remedy for menstrual pain in TCM, but it only works for certain types of dysmenorrhea and is ineffective or even harmful for other kinds of cramps.

- Symptoms: Cold pain in the lower abdomen before/during menstruation; pain gets worse when touching cold surfaces, relieved by hot compress; cold hands and feet; dark red period blood with large clots; delayed menstruation. Dong quai + ginger is the ideal combination.
Type 2: Blood-stasis dysmenorrhea from stress/stagnation of liver qi
- Symptoms: Distending pain in lower belly, breast tenderness before period; mood irritability; irregular cycle; lumpy menstrual flow.
Type 3: Blood-deficiency mild dysmenorrhea

- Heat-type dysmenorrhea Symptoms: Burning pain in lower abdomen, heavy bright-red menstruation, early periods, yellow leucorrhea, dry mouth, easy irritability, hot flashes. Dong quai’s blood-activating property will increase menstrual volume and intensify heat and pain.
- Heavy menstrual flow with no clots Dong quai accelerates blood circulation, leading to excessive bleeding, prolonged menstruation, anemia.
- Cramps caused by organic lesions Uterine adenomyosis, severe endometriosis, uterine fibroids. Dong quai only relieves superficial pain temporarily, accelerates bleeding and aggravates the underlying disease.
4. Safe ways to use dong quai for menstrual cramps
Recipe 1: Dong Quai Ginger Red Date Tea (cold womb cramp)
Ingredients: Dong quai 3g, old ginger 3 slices, red dates 3 pieces, brown sugar.
Method: Simmer for 15 minutes.
Timing: Start drinking 3–5 days before menstruation, stop once menstruation arrives.
Recipe 2: Dong Quai Chicken Soup (blood deficiency cramp, dull pain after period)
Dong quai 5g, astragalus 6g, chicken, simmer weekly after menstruation finishes to nourish blood and prevent next month’s cramps.
