Drug Formulation

2 liang of rhubarb, 1.5 liang of Phellodendron bark, 1 liang of Scutellaria root, 1 liang of Angelica sinensis root (tail), 6 qian of Euphorbia, 6 qian of Datura flower, 5 qian of Euphorbia esculenta, 2 liang of Sanguisorba root, 1 liang of Sophora japonica pods, 1 liang of Saposhnikovia root, 1 liang of raw Rehmannia root, 1 liang of Forsythia fruit,2 liang of Scutellaria baicalensis, 5 qian of Croton tiglium, 1 liang of white arsenic, 5 qian of dragon’s blood, 1 liang of frankincense, 1 liang of myrrh, 1 liang of white alum, 1 liang of fragrant ink, and 100 flower spiders (extract the juice using hemp cloth and add it to the medicinal solution).

Processing

Decoct all the ingredients in water, adding raw white hemp thread to the pot and boiling together until the liquid is reduced to nothing; then remove the hemp thread (the last seven ingredients are used when extracting the pulp from the thread).

Instructions for Use

Disinfect the area and wipe it clean. Gently insert a silver wire with a ball-shaped tip into the fistula tract through the external opening, guiding it so that it emerges from the external opening. When the silver wire passes through the internal opening of the fistula into the rectum, immediately insert a hook into the anus to grasp the silver wire. Pull outward; the silver wire will bend, and it can then be pulled out of the anus via the hook;Tie one end of the medicated thread to the ball-shaped tip of the silver wire, then slowly pull the silver wire out through the original external opening, positioning one end of the medicated thread at the external opening of the fistula and the other end outside the anus;Cut the connection between the medicated thread and the silver wire. Using a “shoemaker’s lacing” technique, loosen the thread near the fistula opening so that the two strands form a single loop. Then, thread the inner end through the loop and slowly pull the medicated thread out through the anal opening from the inner end, creating a double thread. Tie a single knot at both ends of the thread to secure it tightly against the fistula opening, leaving approximately 5–6 centimeters of medicated thread protruding;Finally, tie the two outer ends together with a secure knot, apply an anti-inflammatory ointment externally, and secure it with adhesive tape. After the thread has been in place for 2–3 days, check the affected area for signs of inflammation; if none are present, cut off the secure knot at the end of the thread (leaving the original single knot intact) until it falls out completely. Apply anti-inflammatory ointment to the wound surface.
